What is OPWDD?
The Office for People With Developmental Disabilities (OPWDD) is the New York State agency responsible for coordinating services for more than 128,000 New Yorkers with developmental disabilities. Their mission is to help people with developmental disabilities live richer lives that include meaningful relationships, good health, personal growth, and a home within their community.

What is the Front Door Process?
Step 1: Contact OPWDD
The process begins by reaching out to your local OPWDD Regional Office to express interest in supports and services.
Step 2: Information Session
Attend a required Information Session to learn about the different types of services and how to navigate the system.
Step 3: Eligibility
Determine eligibility by providing documentation of a developmental disability and meeting clinical and financial criteria.

Watch informative videos or attend a Front Door information session to understand OPWDD services.
Submit documentation to determine if the individual has a qualifying developmental disability.
Identify specific needs, strengths, and goals through a person-centered assessment process.
Choose a Care Coordination Organization (CCO) to assign a dedicated Care Manager.
Work with your Care Manager to develop a Life Plan that outlines requested supports.
OPWDD reviews the service requests and Life Plan for final funding authorization.
Identify and interview agencies or individuals that can provide the authorized services.
Formalize service agreements and begin receiving the authorized support and advocacy.

Help with job discovery, training, and ongoing support to maintain meaningful employment.
Housing Supports
Various options ranging from independent living with supports to residential settings with 24/7 care.
Intensive Behavioral Services
Specialized behavioral assessments and support plans to help individuals manage complex needs.
Family Support Services
Information, training, and crisis support designed specifically for the families of individuals with disabilities.
Assistive Technology
Funding for specialized equipment and technology that improves communication and independence.
Important Information
Our advocacy services are designed to complement—not replace—the services provided by OPWDD, Care Coordination Organizations (CCOs), and other authorized providers.
We do not:
- Determine OPWDD eligibility
- Complete eligibility or psychological assessments
- Conduct Self-Direction assessments or budget development
- Provide Care Management or Fiscal Intermediary services
- Approve or authorize OPWDD services
Instead, we provide:
Personalized guidance, advocacy, education, document organization, and ongoing support throughout the process.
When specialized assessments or official OPWDD services are required, we can connect you with trusted community organizations and professionals who can help you take the next step.
